Physical TherapistEthan earned his undergraduate degree in Kinesiology from Oregon State University and his Doctorate of Physical Therapy from Baylor University in Waco, Texas. He has a strong interest in orthopedic rehab and helping patients return to the activities they love.
Outside of the clinic, Ethan enjoys golfing, running, traveling, and spending time with friends and family. He’s recently engaged and planning a wedding with his fiancée. A lifelong sports fan, he rarely misses an Oregon State football game.

Glen Akers, PT, DPT, is a physical therapist and the Director of the Gresham Clinic. He is also a Partner in ProActive Physical Therapy Specialists.

He is also a Partner in ProActive Physical Therapy Specialists.Glen completed his undergraduate degree at Oregon State University, Masters of Physical Therapy degree from at Texas Tech University, and a Doctorate in Physical Therapy from Evidence In Motion University of Health Sciences. He is a member of the American Physical Therapy Association (APTA).
His areas of specialization include ASTYM, TMJ/TMD dysfunction, work conditioning, physical/functional capacity evaluations, and industrial rehabilitation. Glen is certified to perform job analysis and enjoys helping local companies hire and maintain physically qualified employees utilizing WorkSteps.
He is an active member of the Gresham Rotary and enjoys helping local students, YMCA Camp Collins, and delivering Meals on Wheels. Outside the clinic he enjoys cycling, baseball, home brewing, and spending time with his wife and two daughters.

Sean Bennett PT, DPT

Sean received his undergraduate degree from Western Washington University in Bellingham, WA. Originally, from Washington State, Sean moved to Oregon while completing his Doctorate in Physical Therapy through the University of Southern California so he could experience more of the beauty of the Pacific Northwest. His professional interests include vestibular (vertigo) rehab, exercise and wellness, and sharing his passion for human anatomy and physiology with his patients.
Sean earned his Orthopedic Clinical Specialty in 2025 which is the pinnacle of orthopedic certifications for physical therapists; only 6% of PTs have this.
Outside of the clinic, Sean enjoys reading, dancing, fair-weather running, baking, and skydiving.

Clinic DirectorTyler received his Bachelor of Science in Exercise Science from Saint Louis University in 1997 and continued there to receive his Master of Physical Therapy degree in 1999. Tyler furthered his education and completed his Doctorate of Physical Therapy in 2019. He is a Board-Certified Clinical Specialist in Orthopedic Physical Therapy through the American Physical Therapy Association (APTA). He is certified in ASTYM to treat chronic or acute soft tissue and joint conditions
Tyler was a Physical Therapist at ProActive’s Canby location since its opening in 2002 until 2018 when he started ProActive’s Sherwood location. His areas of expertise include extremity and spinal orthopedic conditions, work injuries, post-surgical rehabilitation, athletic injuries, chronic pain, TMJ dysfunction, balance disorders, and fall prevention.
Tyler and his family have lived in Sherwood since 2004. He has enjoyed living in Sherwood and becoming involved with sports and other activities through his children. Tyler has a passion for helping others and enjoys working with members of the community to help them recover from pain and limitations. He is excited to give back to the community he loves. Outside the clinic, Tyler enjoys camping, hiking, gardening, and spending time with his wife and children.

Physical Therapist AssistantRachel received her undergraduate from Shenandoah University in Winchester, Virginia and her PTA license from Blue Ridge College in Martinsburg, West Virginia. Since then, she has carried her career to the West coast with the passion of treating high level athletes, post operative patients and the geriatric population. In her free time, Rachel enjoys running, climbing and traveling for outdoor adventures.

Nicole Cohen earned her Doctor of Physical Therapy (DPT) from Graduate School. Prior to that, Nicole obtained her Bachelor of Science in Athletic Training from Keene State College.
Nicole specializes in orthopedic injury, neurological conditions, and chronic pain and is most interested in treating neurological conditions.
In her free time, Nicole enjoys paddleboarding, hiking, and playing with her dog.

Physical TherapistZoralei has always loved two things: learning and movement! In 2020, she was able to bring these two passions together by completing a Bachelor’s of Exercise Science from Brigham Young University. From there she finished her educational journey closer to home graduating as a Doctor of Physical Therapy from George Fox University!
Alongside treating general orthopedic and neuromuscular conditions, Zoralei specializes in pelvic health. She is passionate about helping women, men, and children navigate pelvic floor dysfunctions with dignity and empowerment.
When she is not at the clinic, Zoralei can be found in four main places. First, outside most likely on a family hike, paddle boarding, or trying to grow something in the garden. Second, on the couch crocheting, gaming on her Xbox, and binging comedy TV. Third, at the gym (probably in a Crossfit class). Fourth, on a date with her husband; eating sushi and solving an escape room.

Physical Therapy AssistantAl DeGuzman earned his physical therapy assistant degree from Western Institute of Science & Health, Rohnert Park, CA. Al has over 25 years working in the profession of physical therapy, orthopedic physical therapy, with 2 years of experience working with hand specialty and splint fabrication.
Al enjoys working with patients post-operatively from total joint replacements, working with patients with knee and shoulder dysfunctions. He has past training with ankle and sports taping, Kinesio Taping.
Outside of physical therapy, Al enjoys activities in the outdoors, jet skiing, playing the acoustic/electric guitar (40+ years of experience) and cooking.

She enjoys treating a wide variety of orthopedic injuries and her areas of interest include balance and fall prevention, older adults, and sports rehabilitation.
Outside of the clinic, Christina enjoys playing volleyball and golf, hiking, cooking and baking, and spending time with family and her dog, Ranger.

In her free time, Andrea enjoys kayaking, hiking, cooking, eating and riding motorcycles.

Outside the clinic, Kyra enjoys hiking, backpacking, playing volleyball, spending time with friends & family, and discovering new restaurants.

Josh received both his undergraduate degree and doctorate in physical therapy from Creighton university in Omaha, NE. He is a board certified geriatric physical therapist. He specializes in care for post-op joint replacement, orthopedics, and sports rehabilitation.
Outside of work, Josh enjoys spending time with his family, traveling, exercising, and woodworking.

Colton Hubler, PT, DPT, is a physical therapist and the Canby Clinic Director.Colton knew that he was going to be Physical Therapist since he was 10 years old after a sports injury. He has always enjoyed sports, being active, helping others, and was inspired by his Physical Therapist at a young age. This led him to earning an undergrad degree from Oregon State University before heading to San Marcos, CA for his Doctorate of Physical Therapy at the University of Saint Augustine.
Colton has worked at PROactive PTS since graduating in 2016 and has been the Clinic Director of the Canby clinic since 2018.
His certifications include:
- Titleist Performance Institute Level 1 & 2, Power Level 2 (Treating Golfers)
- Selective Functional Movement Assessment (Movement Analysis)
- ASTYM (A style of instrumented assisted soft tissue mobilization)
- Lifestyle Medicine Certified (Emphasizing healthy lifestyle habits including Sleep, Nutrition, Stress Management, Exercise, and Social relationships)
- Therapeutic Pain Science (Treating patients with Chronic pain)
He also has advanced training in Manual Therapy, Aquatic Therapy, and treating the TMJ. Other areas of interest include all extremity and spinal orthopedic conditions, post-surgical rehab, worker’s comp, MVA, balance and gait training, and sports rehabilitation for all ages. Colton really enjoys seeing patients progress in their functional ability and learn how to manage their own body.
Outside of being a Physical Therapist, Colton stays busy with his two little girls (Solara and Hayden), big dog (Rogan), and beautiful wife (KC). He continues to play outdoor soccer, golf, and spends the rest of his time with friends and family.

Physical Therapist AssistantJen graduated from Mt Hood Community College in 1989 with an Associate of Science in physical therapy. She has worked in a variety of settings for over 30 years and specializes in manual therapy, orthopedic injuries, geriatric care, and neurological rehab.
She is married with 2 grown children and lives in Redland. Jen enjoys gardening, hiking, camping, and spending time with her husband and 2 dogs.

She enjoys treating a wide variety of orthopaedic injuries, and has a background in strength and conditioning for the geriatric population. Her interests are in manual therapy and functional therapeutic exercise.
In her free time, Keli loves cooking, CrossFit, and traveling.

Clinic DirectorKristine received her Master’s and Doctorate degree in Physical Therapy from Husson University in Bangor, Maine. She has been working as a physical therapist for 20 years and has a specialty certification in Orthopedics (OCS). Kristine specializes in working with all orthopedics injuries (surgical and non-surgical) including neck and back pain as well as vestibular rehab, pregnancy and post-partum care, temporomandibular joint dysfunction and osteoporosis. Kristine focuses on creating individualized treatment plans that not only focus on improving pain, functional and athletic abilities but also prevention of future injuries. She combines specialized manual therapy techniques with custom exercises to achieve her patients’ goals.
Outside of work, Kristine enjoys spending time outdoors with her husband and two sons. She enjoys running, skiing, and camping with her family and friends.
