Glen Akers PT, DPT, Clinic Director
Glen Akers, PT, DPT, is a physical therapist and the Director of the Gresham Clinic. He is also a Partner in ProActive Physical Therapy Specialists.Glen Akers PT, DPT, Clinic Director
Glen Akers, PT, DPT, is a physical therapist and the Director of the Gresham Clinic. He is also a Partner in ProActive Physical Therapy Specialists.Glen completed his undergraduate degree at Oregon State University, Masters of Physical Therapy degree from at Texas Tech University, and a Doctorate in Physical Therapy from Evidence In Motion University of Health Sciences. He is a member of the American Physical Therapy Association (APTA).
His areas of specialization include ASTYM, TMJ/TMD dysfunction, work conditioning, physical/functional capacity evaluations, and industrial rehabilitation. Glen is certified to perform job analysis and enjoys helping local companies hire and maintain physically qualified employees utilizing WorkSteps.
He is an active member of the Gresham Rotary and enjoys helping local students, YMCA Camp Collins, and delivering Meals on Wheels. Outside the clinic he enjoys cycling, baseball, home brewing, and spending time with his wife and two daughters.
Sean Bennett PT, DPT
Sean Bennett PT, DPT
Sean Bennett earned his Doctor of Physical Therapy (DPT) from the University of Southern California. Prior to that, Sean obtained his Bachelor of Arts from Western Washington University.
Sean specializes in orthopedics and is most interested in treating general weakness, unsteadiness on feet, and balance difficulties.
In his free time, Sean enjoys running, reading, and skydiving.

Sherwood Clinic DirectorTyler received his Bachelor of Science in Exercise Science from Saint Louis University in 1997 and continued there to receive his Master of Physical Therapy degree in 1999. Tyler furthered his education and completed his Doctorate of Physical Therapy in 2019. He is a Board-Certified Clinical Specialist in Orthopedic Physical Therapy through the American Physical Therapy Association (APTA). He is certified in ASTYM to treat chronic or acute soft tissue and joint conditions
Tyler was a Physical Therapist at ProActive’s Canby location since its opening in 2002 until 2018 when he started ProActive’s Sherwood location. His areas of expertise include extremity and spinal orthopedic conditions, work injuries, post-surgical rehabilitation, athletic injuries, chronic pain, TMJ dysfunction, balance disorders, and fall prevention.
Tyler and his family have lived in Sherwood since 2004. He has enjoyed living in Sherwood and becoming involved with sports and other activities through his children. Tyler has a passion for helping others and enjoys working with members of the community to help them recover from pain and limitations. He is excited to give back to the community he loves. Outside the clinic, Tyler enjoys camping, hiking, gardening, and spending time with his wife and children.

Nicole Cohen earned her Doctor of Physical Therapy (DPT) from Graduate School. Prior to that, Nicole obtained her Bachelor of Science in Athletic Training from Keene State College.
Nicole specializes in orthopedic injury, neurological conditions, and chronic pain and is most interested in treating neurological conditions.
In her free time, Nicole enjoys paddleboarding, hiking, and playing with her dog.

Jenny graduated from Mt. Hood Community College in 2005 with an Associate of Applied Science degree in Physical Therapy. She is Graston Technique certified.
Jenny’s areas of interest include work conditioning, orthopedics, cupping, balance disorders, and geriatric conditioning.
Outside the clinic, Jenny enjoys playing volleyball, traveling, going on cruises, and scuba diving.

Physical TherapistZoralei has always loved two things: learning and movement! In 2020, she was able to bring these two passions together by completing a Bachelor’s of Exercise Science from Brigham Young University. From there she finished her educational journey closer to home graduating as a Doctor of Physical Therapy from George Fox University!
Alongside treating general orthopedic and neuromuscular conditions, Zoralei specializes in pelvic health. She is passionate about helping women, men, and children navigate pelvic floor dysfunctions with dignity and empowerment.
When she is not at the clinic, Zoralei can be found in four main places. First, outside most likely on a family hike, paddle boarding, or trying to grow something in the garden. Second, on the couch crocheting, gaming on her Xbox, and binging comedy TV. Third, at the gym (probably in a Crossfit class). Fourth, on a date with her husband; eating sushi and solving an escape room.

Physical Therapist AssistantAndrea Fastenow earned her associate degree from Mt. Hood Community College. Andrea enjoys treating a wide variety of orthopedic injuries and her areas of interest include vestibular and balance disorders, fall prevention, concussion rehab, stroke rehab and post-op.
In her free time, Andrea enjoys kayaking, hiking, cooking, eating and riding motorcycles.

Kristin is most interested in treating sports rehab or post-op.

She enjoys treating a wide variety of orthopedic injuries and her areas of interest include industrial & sports rehabilitation, balance & fall prevention and pain neuroscience & education.
Outside the clinic, Kyra enjoys hiking, backpacking, playing volleyball, spending time with friends & family, and discovering new restaurants.

Josh received both his undergraduate degree and doctorate in physical therapy from Creighton university in Omaha, NE. He is a board certified geriatric physical therapist. He specializes in care for post-op joint replacement, orthopedics, and sports rehabilitation.
Outside of work, Josh enjoys spending time with his family, traveling, exercising, and woodworking.

Colton Hubler, PT, DPT, is a physical therapist and the Canby Clinic Director.Colton received his undergraduate degree from Oregon State University in Exercise and Sports Science and his Doctorate of Physical Therapy from the University of Saint Augustine in San Marcos, CA. He is certified in Selective Functional Movement Assessment (SFMA) and has advanced training in manual therapy. Colton is currently preparing for the Orthopedic Clinical Specialist (OCS) certification exam. He specializes in the areas of manual therapy, extremity and spinal orthopedics, chronic pain, and sports rehabilitation.
When he’s not at work, Colton enjoys traveling and hiking with his fiancé, KC, and dog, Rogan, playing sports, and spending time with friends and family.

Kristen earned her Associate of Applied Science at Laramie County Community College in Cheyenne, WY in 2016 and is a Certified Kinesio Taping Practitioner. She enjoys treating a wide variety of orthopedic conditions with focus on manual therapy, therapeutic exercise and education.
She moved here from Wyoming in July 2018 and in her free time enjoys exploring the PNW with her husband and two children. She enjoys roller skating, bowling, hiking and hopes to one day start an animal rescue.

Keli Koistinen is a Physical Therapist Assistant at ProActive’s West Vancouver clinic.Keli received her Associates of Applied Science Degree from Lake Area Technical Institute in Watertown, South Dakota.
She enjoys treating a wide variety of orthopaedic injuries, and has a background in strength and conditioning for the geriatric population. Her interests are in manual therapy and functional therapeutic exercise.
In her free time, Keli loves cooking, CrossFit, and traveling.

Physical Therapist AssistantJenn graduated in 2006 from Mt. Hood Community College with her Associates of Applied Science degree in Physical Therapy and her Bachelor of Applied Science degree in Health from Arizona State University in 2020. Since graduating in 2006, she has worked for Proactive PTS traveling. She has received recognition of Advanced Proficiency in Orthopedics from the American Physical Therapy Association, and has earned certifications for Kinesio Taping, ASTYM, and CrossFit Level-1. She specializes in pelvic health, TMD, vestibular and post-concussion injuries but enjoys treating people with all types of aches and pains.
Outside the clinic, Jenn and her husband are busy running after their two boys and dog Ruby. They are involved in their local community and stay busy on the weekend with sports. Her family enjoys skiing, camping and traveling the world together.

Kristine MacLean-Talbot earned her Doctor of Physical Therapy (DPT) from Husson University. Prior to that, Kristine obtained her Bachelor of Science in Health Science and master’s degree in Physical Therapy from Husson University.
Kristine specializes in orthopedics, vestibular, osteoporosis, and postpartum care.
In her free time, Kristine enjoys spending time with her husband and two children and running.

Physical Therapy AssistantCody McCulloch is a Physical Therapist Assistant at the East Vancouver Proactive PTS. He’s been a PTA for 2.5 years and really enjoys working with people in their recovery and helping improve their quality of life physically and mentally. He enjoys playing any sport and/or learning new ones. Currently he’s been playing more ultimate frisbee but football is his main sport with his favorite team being the best team, The Green Bay Packers. He enjoys the outdoors with his friends and loved ones. He’s happily engaged and getting married in January and looks forward to all the adventures they will have together.
